If you are close with your brother have a separate converstation with him about this. My niece has an autisim spectrum disorder and has had treatment since she was a baby. If it wasn't for that I doubt she would be the high functioning young lady she is. Maybe give them some pamphlets or self-questionarres to get them thinking. Early intervention will go far so they shouldn't wait.

I have a 3 year old cousin and he was doing the same things, he was diagnosed with autism about a week ago, you should check out to see if your brothers son matches the symptoms of autism, its at least worth looking into.
