Failing Teen

Hi,

My daughter is close to 16 yrs. old. Since 5th grade she has struggled with most test either failing or passes with minimum score. The only reason she has been passing the grade is because of class participation, homework or tutors. I thought she just doesn't try enough but now she is trying to get her driving permit and has failed the test 4 times already. I had her take a WISC IV and her IQ was 83 but perceptual reasoning 69. On the educational she scored in the 90's. I am a teacher and believe something is wrong but not sure what or how to help her.
